Rabipac DSR (also known as Rabipac-D SR Capsule) is a combination medication primarily used to treat Gastroesophageal Reflux Disease (GERD), commonly known as acid reflux, and peptic ulcer disease. It works by addressing both the excessive acid production and the movement of food through the digestive tract.

Here's a breakdown of its key components and functions:

Ingredients:

• Rabeprazole: This is a proton pump inhibitor (PPI). It works by reducing the amount of acid produced in the stomach. It specifically blocks the H+/K+ ATPase enzyme (also known as the gastric proton pump) located in the stomach wall, which is responsible for secreting gastric acid.
• Domperidone: This is a prokinetic agent (also known as a dopamine antagonist). It acts on the upper digestive tract to increase the movement of the stomach and intestines. This helps food move more easily through the digestive system and also blocks the vomiting-inducing center in the brain, thereby relieving nausea and vomiting.

Uses:

Rabipac DSR is prescribed for conditions where excessive stomach acid and/or impaired gut motility cause discomfort, including:

• Gastroesophageal Reflux Disease (GERD) / Acid Reflux: It helps relieve symptoms like heartburn, stomach pain, irritation, and regurgitation by reducing acid and facilitating gas passage.
• Peptic Ulcer Disease: It reduces stomach acid, allowing stomach or intestinal ulcers to heal and preventing further damage.
• Zollinger-Ellison Syndrome: A rare condition where a tumor causes excessive stomach acid production.
• Nausea and Vomiting: Domperidone specifically helps with these symptoms.

Mechanism of Action:

Rabipac DSR works synergistically:

• Rabeprazole reduces the "source" of the problem by significantly decreasing the amount of acid secreted by the stomach.
• Domperidone improves the "flow" by enhancing gastric emptying and gut motility, preventing acid and food from refluxing back into the esophagus and relieving associated symptoms like bloating and discomfort.

Important Considerations and Potential Side Effects:

• Dosage: It's typically taken once daily, usually an hour before a meal (preferably in the morning), and swallowed whole.
• Common Side Effects: Headache, diarrhea, nausea, abdominal pain, vomiting, flatulence, dizziness, dry mouth, and weakness.
• Long-Term Use: Prolonged use (over a year) may increase the risk of:
  • Bone fractures (osteoporosis): Due to decreased calcium absorption.
  • Vitamin B12 deficiency: Due to reduced acid-dependent absorption of B12.
  • Low magnesium levels (hypomagnesaemia).
• Precautions:
  • Inform your doctor if you have kidney or liver disease.
  • Not generally recommended for pregnant or breastfeeding women.
  • Can cause dizziness and sleepiness, so avoid driving or operating machinery until you know how it affects you.
  • Avoid alcohol, as it can worsen acidity and drowsiness.
  • May interact with other medications, so always inform your doctor about all medicines, supplements, and herbs you are taking.
  • If symptoms persist after 14 days, consult your doctor.
  • Rarely, it might mask symptoms of gastric cancer, so report severe stomach pain or gastric bleeding immediately.
